• MOSAIC Medicine will typically do serum testing through Vitality Wellness labs, if we choose a different company, we’ll discuss at our visit.
• MOSAIC Medicine will create your Vitality Wellness order
  • YOU will get an email from them confirming your mailing address - please watch for this
  • Your paper order will be emailed to you from MOSAIC Medicine. You will take this paper with you to your lab draw appointment.
• Vitality Wellness will MAIL you the collection kit. The kit will arrive with ice packs - open kit and remove ice packs and freeze them; store your kit as directed until the day of collection. It is ok to store at room temperature until the day of the testing. 
• Take your kit, the frozen ice packs and your paperwork with you on the day of lab collection
• IMPORTANT Notes for scheduling:
  • Blood work may be fasting or non-fasting. MOSAIC Medicine will make this clear on your order and at your visit so you know if you can eat prior to lab draw.
  • Drink plenty of water before your labs for best access
  • If MOSAIC is testing your sex hormones, for cycling/premenopausal or perimenopausal women, have blood work drawn during days 19- 22 of your cycle (Day 1 is the first day of period/bleeding).
  • For non-cycling or postmenopausal women, or men, bloodwork can be collected on any day of the month.

• When results are completed, MOSAIC Medicine will discuss with you at your scheduled followup visit or contact you to schedule the visit.

• MOSAIC Medicine will complete the testing request through RUPA Health
  • You will get an email from RUPA to verify your mailing address - please watch for this and confirm your information
• RUPA Health will process your order and mail you the ordered/appropriate collection kit. The kit will be from lab companies that MOSAIC Medicine uses including: Genova, ZRT, Precision Analytic (DUTCH), Microbiome labs
• On arrival of test kit, open box and review test kit instructions for test preparation and collection
  • There may be some medications (ie, probiotics, NSAIDS, H2 blockers or PPIs, etc) to avoid for a time period before collecting stool sample. Please reach out if you have questions about holding these medications.
• Read the directions for how to collect sample - please reach out if questions
• Plan ahead and identify the day(s) you will collect your sample
• On the day(s) of your collection, follow package insert and instructions. All needed supplies will be in the kit.
• After collection, follow package instructions for shipping your specimens back to the lab
• Results will automatically be sent to MOSAIC Medicine and can take up to 10 days. Once all results have come in, we will discuss the results with you at your scheduled followup visit or we will contact you to get scheduled. 

• MOSAIC Medicine will complete the testing request through RUPA Health

• RUPA Health will process your order and mail you the ordered/appropriate collection kit. The kit will be from lab companies that MOSAIC Medicine uses including: Genova, ZRT, Precision Analytic (DUTCH), Microbiome labs
• On arrival of test kit, open box and review test kit instructions for test preparation and collection
  • For breath testing, there is an important diet preparation the day (24 hours) prior to collection followed by a fasting period (8-12 hours) the day of collection.
    • Best scenario: diet preparation 24 hours prior to scheduled collection, eat dinner, fast overnight and complete the test in the morning.

• Plan ahead and identify the day(s) you will collect your sample
• On the day(s) of your collection, follow package insert and instructions. All needed supplies will be in the kit.
• After collection, follow package instructions for shipping your specimens back to the lab
• Results will automatically be sent to MOSAIC Medicine and can take up to 10 days. Once all results have come in, we will discuss the results with you at your scheduled followup visit or we will contact you to get scheduled.

• For saliva testing, please read instructions for medications or foods to avoid prior to testing
  • There are specific instructions for if you are a patient taking hormone replacement therapy/menopausal hormone therapy - please review
  • Hold any retinols and anti-wrinkle/aging creams for 3 days
  • Avoid eating, drinking or brushing teeth up to 2 hours prior to collecting any sample throughout the day
  • Fill all 4 tubes per collections instructions.
    • Tips to help with saliva production include smelling food, yawning, thinking of something sour, or pressing your tongue against your teeth or roof of your mouth.

• For urine collection of hormones:
  • For cycling/premenopausal or perimenopausal women, collect urine during the days 19-22 of cycle (Day 1 is the first day of period/bleeding).
  • For non-cycling or postmenopausal women, or men, dried urine can be collected on any day of the month.
• Review “Collection Rules” and “What to Avoid” sections in package insert specifically for caffeine, alcohol, exercise, food and drink as these can affect test results.

• On the day(s) of your collection, follow package insert and instructions. All needed supplies will be in the kit.
• After collection, follow package instructions for shipping your specimens back to the lab
• Results will automatically be sent to MOSAIC Medicine and can take up to 10 days. Once all results have come in, we will discuss the results with you at your scheduled followup visit or we will contact you to get scheduled.

• All tests and collection fees have been pre-paid through MOSAIC Medicine using your card on file at the clinic
• There will be a $100.00 "MOSAIC Medicine lab and administrative" fee on all lab orders
