1 Service Description
Our SMS service provides appointment reminders, health tips, and important practice updates.
2 Acceptance of Terms

3 Messaging Frequency
You will receive up to 10 messages per week regarding appointment reminders, special promotions, updates, and other relevant communications. The frequency may vary depending on upcoming events or important announcements.
4 Opt-in Process
To subscribe to our SMS alerts, text “JOIN” to 605-503-8787. Upon successful subscription, you will receive a confirmation message:“You’ve successfully subscribed to MOSAIC Medicine SMS alerts.”
5 Opt-out Process
You may opt-out of receiving SMS messages at any time. To unsubscribe or opt-out, simply reply with the word “STOP” to 605-503-8787.
Upon opting out, you will receive a confirmation message indicating that you have been unsubscribed and will no longer receive messages from MOSAIC Medicine.

6 User Obligations

12 Governing Law and Jurisdiction
This Agreement shall be governed by and construed in compliance with the domestic laws of the State of South Dakota without giving effect to any choice-of-law or conflict-of-law provision or rule (whether of the State of South Dakota or of any other jurisdiction) that would cause the application of the laws of any jurisdiction except the State of South Dakota.
